RCA Lifecycle
There are two ways a Root Cause file can be created. The first way is through a Root Cause Analysis submission form (Submitting an RCA File). The second way is to create it using an existing file (linking the two files). For example, once it is determined that an adverse event requires an investigation, File Managers can create RCA files from the event Management Form using the Send to Another Module option (Sending Data to Another Module).
